Deep or Surface? How Classical Chinese Medicine Tracks Your Knee Pain

When you experience joint discomfort, it’s easy to treat it as a simple mechanical issue. But almost 2,000 years ago, the master physician Zhang Zhongjing revolutionized medicine with his theory of the Six Stages of Disease Progression. He mapped out how illness travels—starting as a superficial obstruction on the outside of the body and, if left unchecked, sinking into the deep, foundational layers of our internal systems.

In my practice, knee pain isn’t just knee pain. To treat it effectively, I have to figure out exactly which of the Six Stages your body is currently navigating. Let’s look at how the same joint requires a completely different approach based on how deep the condition has penetrated.

## Scenario A: The Traumatic Stagnation (The Taiyang Stage)

Imagine someone who suffers a sudden knee injury—like a twist on a sidewalk, a sports collision, or a car accident. The joint is swollen, bruised, sharply painful to touch, and stiff.

* The Classical Stage: This sits at the Taiyang (Greater Yang) stage. Because it is a recent trauma, the issue is locked in the body’s superficial layers—the skin, muscles, and defensive channels. The body’s energy is strong, but it is trapped and struggling to move past the injury site.

* The Treatment Plan: Since the pathogen (the trauma) is on the outside, we use aggressive, outward-moving strategies to break the blockage.

* Acupuncture: We focus heavily on local acupuncture points directly encircling the joint—such as Xiyan (Eyes of the Knee) and ST35—to violently trigger local blood circulation and drain the swelling.

   * Herbal Therapy: Because the issue is at the surface, we use external classical remedies. Applying a traditional Dit Da Jow (liniment) directly to the skin helps break up the stagnant Blood and Qi right at the superficial layer.

## Scenario B: The Deep Wear-and-Tear (The Shaoyin Stage)

Now, picture a completely different struggle: a person with a long-term, dull, aching knee. There was no accident. Instead, the pain is a weak, cold ache that gets worse when they are exhausted or when winter weather hits New York. This is common in chronic conditions, poor diet depletion, or long-term illness recovery.

* The Classical Stage: This pain has sunk all the way down to the Shaoyin (Lesser Yin) stage. At this deep level, the body’s essential internal “fire” (Yang energy) has been depleted. The Liver and Kidneys—which govern the tendons and bones—no longer have the resources to nourish the joint. The knee is aching because the core battery of the body is running low.

* The Treatment Plan: Rubbing liniment on the skin won’t fix a deep, internal shortage. Sweating or heavy local manipulation would only exhaust the patient further. We must warm the core and rebuild the foundation.

* Acupuncture & Moxa: We combine those local knee points with powerful systemic, root-regulating points like KI3 (Taixi) to wake up the Kidney energy. We also use Moxibustion (heat therapy) to deeply warm the channels from the inside out.

   * Herbal Therapy: We use internal, deeply warming classical formulas—such as Du Huo Ji Sheng Tang—to specifically target Shaoyin cold-dampness, nourish the bones and tendons, and restore the vital energy lines at their deepest root.
